🌱 Sustainability and responsible use of medication

1. Contact, assessment, and getting to know the person

Care begins before medication.

At FarmAssist, nothing is prepared without truly knowing the person. The first step is a complete assessment, carried out by a pharmacist, to ensure clinical safety, therapeutic suitability, and human understanding.

What is done at this stage:

  • Direct contact with the person or caregiver

  • Structured collection of clinical information

  • Analysis of current medical prescriptions

  • Definition of who uses the service and who manages it

  • Clear explanation of how it works and continuous monitoring

2. Preparation (PIM) at your pharmacy

Technology and pharmacists working together.

The medication is prepared at a partner pharmacy, using individualized preparation systems and professional validation, always with coordination between teams.

The technology used in individualized preparation is always complemented by validation and monitoring by pharmacists. The combination of dosing systems and clinical knowledge allows for greater accuracy, lower risk of error, and safer and more consistent treatment monitoring.

What is done at this stage:

  • Individualized preparation by dose, day, and time

  • Use of specific PIM technology

  • Multi-stage pharmaceutical validation

  • Continuous communication between pharmacists at the pharmacy and Farmassist
